NEW STUDENTS

​Required Documents

​

1. Two proofs of residency (all grades). Any of the following is acceptable proof of residency:   

a. a deed,

b. a lease,

c. current utility bill,

d. current credit card bill,

e. property tax bill,

f.  vehicle registration,

g. driver's license, or a

h. Dept. of Transp. ID card.

2. A Completed child's physical and immunization form.

3. ​A completed Child’s Dentist report (click here to download the                form).

 

4. A copy of the child's birth certificate (in English).

5. If the child is 4-5 years of age: ​ download this form Kindergarten            Teacher Recommendation, give it to your child’s previous teacher         to fill, then upload the completed form on TADS.

6. If the child is 6 years of age or older: download this form Teacher            Recommendation, give it to your child’s previous teacher to fill,               then upload the completed form on TADS.

7. If the child is 6 years of age or older: download this Request for     Transcript, fill it out yourself and mail it to your child’s previous                 school.​

Please be advised that:
1- Only applications with documents marked as satisfactory/fulfilled are processed and evaluated.

2- For Physical and dental forms: some medical offices may take longer to fill the required forms. Kindly upload a note stating the approximate time of receiving and uploading these forms.
