• Two proofs of residency (all grades). Any of the following is acceptable proof of residency:   

  • A deed,

  • A lease,

  • Current utility bill,

  • Current credit card bill,

  • Property tax bill,

  • Vehicle registration, driver's license, or a Dept. of Transp. ID card. 

 

• ​​Completed child's physical and immunization form.

• ​A completed Child’s Dentist report (email the school for form).

 

• A copy of the child's birth certificate (in English).​

 

• If the child is 4-5 years of age: ​ download this form Kindergarten Teacher Recommendation, give it to your child’s previous teacher to fill, then upload the completed form on TADS. 

 

• If the child is 6 years of age or older: download this form Teacher Recommendation, give it to your child’s previous teacher to fill, then upload the completed form on TADS. 

 

• ​If the child is 6 years of age or older: download this Request for Transcript, fill it out yourself and mail it to your child’s previous school.

• Only applications with documents marked as satisfactory/fulfilled are processed and evaluated.

 

• For Physical and dental forms: some medical offices may take longer to fill the required forms. 

  • Kindly upload a note stating the approximate time of receiving and uploading these forms.
