NEW & RETURNING

  • Two proofs of residency (all grades): Any of the following is acceptable proof of residency:
            a. a deed,
            b. a lease,
            c. current utility bill,
            d. current credit card bill,
            e. property tax bill,
            f.  vehicle registration,
            g. driver's license, or a
            h. Dept. of Transp. ID card.

1- Only applications with documents marked as satisfactory/fulfilled are processed and evaluated.

2- For Physical and dental forms: some medical offices may take longer to fill the required forms. Kindly upload a note stating the approximate time of receiving and uploading these forms.
This is only valid for physical and dental forms.

NEW STUDENTS ONLY

    1- A copy of the child's birth certificate (in                English).

   2A- If the child is 4-5 years of age: ​    OR

   2B- If the child is 6 years of age or older:
  • download this form Teacher Recommendation, give it to your child’s previous teacher to fill, then upload the completed form on TADS.

   3- If the child is 6 years of age or older:
  • download this form Request for Transcript, fill it out yourself and mail it to your child’s previous school.​​​
